Employment Forms

Employee Information Form: Complete information to include the citizenship question, omitting the employee number. The address you list on this form is the address to which your check will be mailed. We also require a copy of your Social Security card for payroll purposes; your W-2 will read as it appears on your card.

Employment Eligibility Verification Forms

I-9 Form: Complete Section 1 by first day of work; be sure to include signature and date. This form requires identification, and the choices are listed with the form; IDs are to be provided by the third day of work.

Foreign National Information System (FNIS) Form: As you complete the I-9 form, please note that if you are not a citizen or national of the United States, you will need to complete the FNIS Data Gathering Form.

Payroll Forms

Federal & State Tax Forms: Complete the W-4 and NC-4 forms by signing and dating as well as selecting a marital status and number of allowances 

Direct Deposit Enrollment/Change Form: This form allows direct deposit and also allows employees currently enrolled in direct deposit to view their payroll advices through WebAdvisor. All full-time employees are required to have direct deposit. All part-time employees are eligible for direct deposit. A voided check must be attached for verification of up to three (3) accounts for direct deposit.

Other Forms

Transcript(s): Are required if a degree is required for the position. Please reference the Tips for Requesting Transcripts page for the mailing information. 

NC State Health Plan: All full-time employees receive individual medical insurance through the North Carolina Teachers and State Employees Health Plan. Eligible dependents may be insured at an additional cost through payroll deduction. Visit www.shpnc.org for information regarding available employee and eligible dependent plans. Benefits become effective the first of the month following employment.
